What Is an All-on-4?
An All-on-4 is suitable for patients who have lost all or most of the teeth in either one or both of their jaws. This procedure replaces all of the teeth with a complete arch of new teeth that’s held in place by just four dental implants.
The procedure was pioneered by Dr Paulo Malo , a renowned implantologist from Portugal. It has proven so successful that it’s now taught throughout the world to implant specialists following Dr Malo’s exact protocol.
Am I Suitable for an All-on-4?
Bone Loss
One of the major causes of tooth loss is bone loss in the jaw . This is typically caused by periodontitis , a form of gum disease that has advanced so much that it’s not only the health of the gums that is compromised but also the health of the jawbone too. The disease compromises the tissues that support the teeth, causing so much erosion that teeth may eventually fall out.
Bone loss can happen anywhere in the jaw, and it makes it difficult to place implants if there is not sufficient bone there to hold them—certainly when it comes to traditional, single dental implants.
However, even for patients who have suffered bone loss, an All-on-4 may be a suitable option. This is because four dental implants are inserted in the jaw in areas with sufficient bone. Plus, the rear implants are longer than normal and positioned at a 45-degree rather than 90-degree angle, which means there is greater bone to implant contact, which increases stability.
Smoking
Smokers are not good candidates for dental implants. Smoking reduces blood flow, decreasing the amount of nutrients and oxygen to the cells that fight infection and promote healing. While this is a major concern just after your implant surgery, it is also an issue throughout the life of your implants; this, if you do smoke an All-on-4 is unlikely to be suitable .

What Does the Procedure for an All-on-4 Entail? How Long Will I Need to Stay in Tunis?
Your chosen implantologist in Tunis will discuss with you how long you will need to stay in Tunis in order to complete the procedure. It’s worth remembering that although the actual All-on-4 surgery is carried out in just one day (taking around 2.5 hours per arch of teeth), there is some planning that needs to take place before the surgery.
The treatment includes an examination, treatment planning and placement of the implants. Thus, diagnostic tests, including 3D/CT scanning, are carried out and your dentist will perform a thorough evaluation and create a tailor-made treatment plan. Impressions will need to be taken of your mouth, and these are sent to a laboratory where your new teeth will be made, which will be available for your dentist to place on the day of your surgery.
Do bear in mind that your initial set of prosthetic teeth will be lighter than your permanent ones and are fitted temporarily to give the tissue in your mouth time to heal, allowing the implants to fuse with your jawbone.
You will need to go back to Tunis after a number of months to have your permanent arch of teeth attached to your implants; your dentist will advise you as to when you should be planning your return trip.
Teeth Extractions
All-on-4 treatment is only suitable if you have no teeth in your jaw. So, if you have any remaining teeth they will need to be removed, and your dentist will carry this out at the same time as your All-on-4 surgery.
Is the All-on-4 a Good Procedure for Dental Tourists?
The All-on-4, like most dental procedures, is a good procedure to have as a dental tourist. Although the All-on-4 is a major dental procedure, most patients experience very little in the way of downtime or after-effects following their surgery and can enjoy a holiday and getting out-and-about after a day or two.
